This is the most expensive object on this site and the one most likely to go unused. It is worth being blunt about both facts before the specifications.
Why 48 inches is a safety figure
A wheel bends the running surface under the animal. The tighter the diameter, the steeper the arc a cat's body has to follow while running.
One Fast Cat publishes 48 inches of diameter and a 135 inch circumference. A 15 inch cat occupies about 11 percent of the loop, which is a shallow arc — close enough to flat that the cat's gait is close to natural.
Halve the diameter and the same cat occupies 22 percent of the loop, running through a visibly curved posture. That is the reason to be wary of the cheaper small-diameter wheels, and it is a geometry argument rather than a brand preference.

An unusually complete spec sheet for this category -- diameter, running surface, weight limit and assembly time are all published.
The EVA foam surface is worth noting. Carpet on a wheel catches claws; foam gives grip without something to hook into, which matters at speed.
The honest risk
Plenty of cats never use one. The cats that do tend to be the ones that already sprint the length of the hallway at 3am — and the Ohio State Indoor Pet Initiative frames play as a predatory need with a shape: stalk, chase, catch, kill. A wheel serves the chase phase brilliantly and the other three not at all.
- A cat that stalks and pounces wants cover and a wand, not a treadmill.
- A nervous cat will not commit to a surface that moves under it.
- An older or overweight cat is a veterinary conversation before an exercise-equipment one.
- A flat with neighbors below should factor in the rolling noise before the purchase, not after.
What to try first, at a fraction of the cost

If you do buy one
- Assemble it and leave it stationary for several days. No pressure at all.
- Lure with a wand along the inside surface while it is still.
- Let it turn slowly under the cat's own movement.
- Never place a cat on a moving wheel and never confine one to it.
- Put a rug underneath. It cuts rolling noise and it stops the base creeping.

At 48 inches of diameter the 135 inch circumference means a 15 inch cat occupies about 11 percent of the loop, which is a shallow arc. Small-diameter wheels sold cheaper are the ones to be wary of, and this is why.

36 inches of rod and 32 inches of string is 68 inches of working radius, so the lure can travel a genuine arc across a room rather than bobbing in front of the cat. Short wands produce short, unconvincing movement, which is why cats lose interest in them within a minute.
